Club Undertaking Format for Indian Premier League Complimentary Tickets

As per the instruction of Committee of Administrators (COA), the affiliated clubs should submit an undertaking in the prescribed format on clubs letterhead while collecting complimentary passes for Indian Premier League matches to be played at Wankhede Stadium.
The person collecting complimentary passes for the club(s) are also requested to submit photocopy of any Government approved identity card for the purpose of verification.
